Introduction

Joint replacement is now-a-days one of the commonly performed orthopaedic operations. The most common reason for joint replacement is Osteoarthritis (OA).

Arthritis

Arthritis is a general term covering numerous conditions where the joint surface (cartilage) wears out. This surface can wear out for a number of reasons, often the definite cause is not known. When the articular cartilage wears out, the bone ends rub on one another and cause pain. There are numerous conditions that can cause arthritis and often the exact cause is never known. In general, but not always, it affects people as they get older (Osteoarthritis).

 

Other causes of cartilage destruction

  • Rheumatoid arthritis (RA)
  • Haemophilia
  • Seronegative arthritis
  • Avascular necrosis
  • Gout
  • Obesity
 

Diagnosis

The diagnosis of osteoarthritis is made on history, physical examination & X-rays. There is no blood test to diagnose Osteoarthritis (wear & tear arthritis).

 

Major Clinical Indications for Joint Replacement:

Pain

Activity aggravates the pain while rest relieves it. Pain during the night is particularly distressing to patients and an important surgical indication.

Functional limitations

Walking and particularly climbing the stairs become difficult, with start up pain a particular feature.

Stiffness

There are certain patient subgroups in which joint stiffness without pain is an indication for surgery.

Age

While most hip & knee replacements are performed in patients between 60 and 80 years of age, older or younger age is not a contraindication to surgery.

Early discharge is encouraged, but only if

  • Wound healing is satisfactory
  • Mobility is satisfactory
  • Reasonable degree of movement achieved
  • No complications have been identified
